Swing Bed Intermediate Care

Purpose

Our staff is dedicated to providing the best possible care for our patients. Timely intervention is important and our medical staff members are immediately available to see the patient, regardless of patient level of care.

Who is a Swing Bed Patient

• Patient can be of any age
• A person who needs assistance with the activities of daily living
• A person can use this service while alternate arrangements are being made, either for transfer to a nursing home or return to the community with the use of available resources to help the patient function as fully as possible.

Family Involvement

Family involvement has a positive impact on patient’s mental and physical status and is very important to patient well-being. Therefore, families are invited to attend Care Team Meetings, help volunteer and assist in the many social events conducted through-out the year, as well as participate in the daily activities.
